Documents we need
Funeral Cash Plan
- Claim form.
- Certified copy of the death certificate.
- ID of the person who died.
- ID of the claimant.
- Proof of relationship, where needed.
Hospital Cash Back Plan
- Claim form.
- Hospital records.
- ID.
A claim is complete when we have every document we need. We will tell you straight away if anything is missing.
